Chailey Heritage Pathways

Chailey Heritage Pathways will offer highly specialist care for children, young people and young adults with physical disabilities and/or sensory impairments and associated health needs.

We provide a flexible way to access highly specialist care and support, both at home, or in the community, facilitating access to activities, events or attractions. 

We are very proud of the warm, caring and professional relationships our staff build with children and young people and look forward to widening our reach into the community.

Who We Help

Chailey Heritage Foundation is recognised as one of the UK’s leading centres for children and young adults with neurological motor impairment such as Cerebral Palsy. Chailey Heritage Pathways channels all of that experience, expertise and commitment into the wider community.

How We Can Help

We will provide community support whenever and wherever needed. This could range from a few hours to all day, at weekends, evenings, before or after school and during school holidays.

Our team of highly skilled support workers will work closely with individuals, family members and other professionals to deliver a person centred package of care and support. Each team member is sibject to a a robust training programme by a registered nurse for carers, with competency framework to ensure each individual's needs are met, this will include: 

  • Enteral feeding e.g. PEG/JEJ
  • Epilepsy & emergency medication
  • Ventilation
  • Suction
  • Oxygen
  • Medication

We will also look to develop skill sets in order to meet further or changing needs.

How it works

Packages are bespoke to an individual's needs and can include assistance with personal care, medication, assisted feeds or escorting to medical appointments.

Step 1: An initial assessment, discussing individual needs, hobbies and interests and aspirations for the future.

Step 2: Develop a bespoke package to meet these.

Step 3: Matching the right support team to the individual and package, also taking into consideration any shared interests.

Step 4: Package commences, and regular reviews scheduled.

An update from Pathways

We are delighted that CQC have awarded us an overall quality rating of ‘Good’ for the Pathways provision. You can download the report in full at the bottom of this page. Working in partnership with

Chailey Heritage Foundation works in partnership with Chailey Clinical Services, part of Sussex Community NHS Foundation Trust. They provide medical intervention, clinical expertise and a range of therapies on-site for our children, young people and young adults.
