Pathways offers before and after school support and home care and provide supported living as a post 25 option. We are committed to building packages which are totally built around individuals, with teams recruited to match not only the care needs but also the personality and interests of the young person and family.
As part of Chailey Heritage Foundation, Pathways can access a unique network of specialist support and resources enabling us to support even the most complex of needs. This includes Speech & language Therapy, Massage, Music Therapy, Bespoke Epilepsy Training, Nurse Specialists and Positive Behaviour Support.
Pathways offers truly bespoke personal support. Each individual has their support team tailored directly to to their needs, their personality and their interests and is backed by the knowledge, experience and resources of Chailey Heritage Foundation.
Offering bespoke personal support, our expert teams are committed to creating packages that are shaped around you as an individual. Our highly skilled staff include clinical support from registered health practitioners, allowing us to deliver personalised support to meet even the most complex of needs.
Expert support workers are recruited to match not only the support needs but also the personality and interests of the young person and their family.
Building and delivering support services requires incredible skill, patience and expertise not only from our frontline staff but also from the clinical specialists we work alongside.
The community services teams have access to a network of highly skilled professionals in Positive Behaviour Support, Speech and Language Therapy, Physiotherapy, Rebound Therapy, Nursing and Intervenors.
Our Senior Leadership Team in the department oversees the work of these individuals and carefully designs input bespoke to each individual and the staff working with them.
